None of that research is specific to aesthetics, so the only figure that matters is your own – and most practices have never measured it.

That is the honest version of an answer the internet usually gives with far more confidence than the evidence supports. Almost every “practices miss X% of calls” statistic in circulation comes from a vendor selling call software, and most of them cite each other rather than an original study. The figures are still useful as a range, but they are not a measurement of your practice.

What makes missed patient calls worth taking seriously in aesthetics is not the benchmark – it is the arithmetic. A single cosmetic consultation can be worth several thousand dollars in surgeon fees alone, so a practice does not need a dramatic miss rate for the annual number to be uncomfortable. Three unanswered calls a day, at ordinary conversion rates, is enough.

What this post covers

What the published data actually says about missed patient calls

Why patient calls go unanswered in an aesthetic practice

What is one missed patient call actually worth?

A worked scenario

Which missed patient calls cost the most?

Does missed-call text-back solve it?

How to measure your own missed patient calls this week

What to do once you have the number

Frequently asked questions

What percentage of patient calls does the average medical practice miss?

How much does a missed patient call cost a plastic surgery practice?

Do patients leave a voicemail if nobody answers?

How many patient inquiries arrive outside office hours?

Is missed-call text-back enough on its own?

How quickly should a practice respond to a new inquiry?

Sources

What the published data actually says about missed patient calls

Here are the numbers most often quoted, and where each one comes from. Read the right-hand column before you quote any of them to a surgeon.

Reported miss rate

Attributed to

What to know about it

42% of calls unanswered

An analysis of roughly 7,000 calls across 22 practices in 18 states, attributed to Physician Growth Accelerator

The most-cited figure in healthcare call marketing. Treat them as a range, not a measurement.

What this means in practice: if someone tells you the average aesthetic practice misses a specific percentage of calls, they are extrapolating. There is no published, aesthetics-specific benchmark for missed patient calls. You have to measure your own, and the last section of this post explains how to do it in a week without buying anything.

Why patient calls go unanswered in an aesthetic practice

Missed patient calls are operational, not a reflection of the front desk team. In most practices the same five causes account for nearly all missed patient calls.

Two calls arrive at once. One coordinator can hold one conversation. The second caller hears a ring-out or a voicemail greeting.

The front desk is in the room. Checking in a patient, taking a deposit or walking someone to a treatment room all take the coordinator away from the phone.

Lunch and shift changes. A predictable daily window where coverage drops and inbound volume does not.

After 5pm, weekends and holidays. Patients research procedures in the evening. Every one of those calls is a missed patient call unless something answers.

Channel splitting. The same person is expected to answer the phone, reply to website chat and clear Instagram DMs. Time spent on one is time not spent on the others.

Demand is also rising. ASPS recorded 1,523,330 cosmetic surgical procedures in 2025, a 7% increase on 2024, with breast augmentation up 11% and facelifts up 13%. More procedures mean more inquiries arriving at front desks that have not grown at the same rate.

What is one missed patient call actually worth?

Work it out with four numbers you already have. The formula is simple:

Missed patient calls per day × working days × share that are new-patient inquiries × consultation booking rate × procedure conversion rate × average case value

For average case value, the most recent published figures from ASPS are the 2024 surgeon fee ranges: breast augmentation $4,575–$8,000, rhinoplasty $7,500–$12,500, tummy tuck $8,000–$13,500, facelift $12,000–$19,000, liposuction $4,300–$7,500. These are surgeon fees only and exclude anesthesia and facility costs, so your real case value is higher. ASPS did not publish a fee table in its 2025 report.

A worked scenario

The following is an illustrative scenario, not a client result or a guaranteed outcome. Substitute your own figures.

Missed patient calls per day

5

Working days per month

22

Missed patient calls per month

110

Share that are new-patient inquiries (assumed 30%)

33

Book a consultation (assumed 25% of those recovered)

8

Proceed to a procedure (assumed 40%)

3

Average case value (assumed $6,000)

$18,000 per month

Annualised

$216,000

Illustrative scenario. Every percentage above is an assumption, shown so you can replace it with your own.

Change any assumption and the number moves substantially – that is the point. Halve every rate and it is still over $100,000 a year. The figure that most practices find uncomfortable is not the total but the input: five missed patient calls a day is a quiet week for many aesthetic practices.

Which missed patient calls cost the most?

Not all missed patient calls carry the same value, and practices that triage by caller type recover more than practices that treat every missed call equally.

First-time inquiry about a surgical procedure. The highest value and the most perishable. This caller is comparing two or three practices and will usually call the next one on the list.

Inquiry from a paid campaign. You already spent money to make this phone ring. A missed call here is the advertising cost plus the lost case.

Existing patient wanting to book an injectable or follow-up treatment. Lower immediate value, but these calls tend to come back, so the real cost is delay and friction rather than a lost patient.

Post-operative question. Little direct revenue, high clinical and reputational weight. These should never sit in a voicemail queue.

Does missed-call text-back solve it?

Partly. Missed-call text-back sends an automatic SMS when a call goes unanswered, which is better than silence – but it moves the conversation to a channel the caller did not choose and still requires a human to pick it up. Speed matters here. The foundational research on inbound response time, published in Harvard Business Review in 2011 by Oldroyd, McElheran and Elkington, found firms contacting a lead within an hour were about seven times more likely to reach a decision maker than those waiting one more hour, and more than 60 times more likely than those waiting a day. That study is cross-industry and over a decade old, but the direction has not been contradicted since.

Voicemail

Missed-call text-back

AI front desk

Answers the call live

No

No

Yes

Works after hours

Message only

Yes

Yes

Collects name, phone and procedure interest

Only if the caller leaves it

Needs a reply

Yes, in the conversation

Can book or hold a consultation slot

No

No

Yes

Covers website chat and Instagram DMs

No

No

Yes

Still needs staff follow-up

Yes

Yes

For escalations and clinical questions

For a fuller comparison, including cost and HIPAA considerations, see our guide to the AI receptionist for plastic surgery and med spas.

How to measure your own missed patient calls this week

You do not need new software to get a reliable number on missed patient calls. Count total inbound calls and total missed patient calls.

Add your Google Business Profile call history. Calls placed from your Google listing are logged there and are almost always first-time inquiries.

Separate after-hours from business-hours missed patient calls. These are two different problems with two different fixes.

Check how many missed callers left a voicemail. The gap between missed calls and voicemails is your silent loss.

Count website chat and Instagram DMs that waited more than an hour. Same problem, different channel.

Divide unanswered by total inbound. That percentage is your real miss rate. Compare it to the table above, then run the scenario arithmetic with your own case value.

Most practices that measure their missed patient calls are surprised twice: once by the after-hours total, and once by how few missed callers bothered to leave a message.

What to do once you have the number

If your missed patient calls are concentrated in business hours, the fix is usually staffing, call routing or an overflow line. If they are concentrated after 5pm and at weekends, no amount of scheduling solves it – something has to answer when the office is closed.

Cosmasol builds AI Digital Agents for aesthetic and plastic surgery practices. The AI Calling Agent answers the phone when the front desk cannot, the AI Website Chatbot and AI Social Media Chatbot handle website and Instagram inquiries, and each is trained on the practice’s own procedures, policies and FAQs so the answers match what the team would say. Inquiries flow into the practice’s existing workflow rather than sitting in a separate inbox, and anything clinical or unusual is escalated to a person. The purpose is to stop missed patient calls becoming lost consultations, not to replace the front desk team.

Related reading: AI answering service for med spas and turning Instagram DMs into consultations.

Frequently asked questions

What percentage of patient calls does the average medical practice miss?

Published estimates range from about 15% to 42%, with 20% to 30% the most commonly repeated range for business hours and 30% or more for solo and small practices.
